Athletic Documents and Forms

  • Due to the COVID-19 pandemic during the 2019-20 school year and the continued stress on the medical community, the Indiana High School Athletic Association, Inc. will waive the provisions of Rule 3-10 for the 2020-21 school year. Effective immediately, valid physical forms presented to member schools on behalf of eligible students during the 2019-20 school year shall be valid during the upcoming 2020-21 school year. Any student who didn't get a physical for the 2019-20 school year would still need to get a new physical. This includes any incoming 6th grade students or any incoming 7th or 8th grade students who didn't turn one in last year.

    Parents and students will be responsible for submitting a HEALTH HISTORY UPDATE QUESTIONNAIRE and CONSENT & RELEASE CERTIFICATE in place of a physical. These forms do NOT need to be signed by a physician.

    Parents and students will still be responsible for submitting all forms in RankOne for the 2020-21 school year. The new RankOne forms can be completed after July 1, 2020.

         If you did NOT submit a physical for the 2019-20 schoolyear, submit a hard copy of a physician completed IHSAA physical to the Athletic Department. This physical must be completed after April 1, 2019 to be valid for the following school year. Click on the IHSAA physical form to view and then print. The student/athlete and the parent fill out the front of the form. The physician fills out and signs the back of the form.
    Middle School Student/Athletes are required to pay a Transportation Fee of $25 per student per athletic season.   Please turn in this Transportation Fee form with your payment.
    If you would like to take your child home after an athletic event instead of having him ride the bus, you will need to complete this permission to travel form and turn it in at the NEMS main office prior to the event.
