NEMS Athletic Department
Due to the COVID-19 pandemic during the 2019-20 school year and the continued stress on the medical community, the Indiana High School Athletic Association, Inc. will waive the provisions of Rule 3-10 for the 2020-21 school year. Effective immediately, valid physical forms presented to member schools on behalf of eligible students during the 2019-20 school year shall be valid during the upcoming 2020-21 school year. Any student who didn't get a physical for the 2019-20 school year would still need to get a new physical. This includes any incoming 6th grade students or any incoming 7th or 8th grade students who didn't turn one in last year.
If NEMS has a physical on file from the 2019-20 year for a student, parents and students will be responsible for submitting a HEALTH HISTORY UPDATE QUESTIONNAIRE and CONSENT & RELEASE CERTIFICATE in place of a physical. These forms do NOT need to be signed by a physician.
Parents and students will still be responsible for submitting all forms in RankOne for the 2020-21 school year. The new RankOne forms can be completed after July 1, 2020.

ALL student/athletes must complete athletic paperwork in order to be eligible to try out for a middle school sport.
Athletic paperwork consists of:1) the IHSAA physical form which must be completed, signed by a physician, and turned in at the NEMS front office or Student Services (as stated above, if you have a physical on file dated after April 1, 2019, this is still valid.)2) the online forms that can be completed and submitted at Rank One Sports
