To request an excused absence due to extraordinary circumstances, use the following form:

Leaving School for a Medical/Dental Appointment
The student is to attend school before and after appointments.
A parent is to call the attendance office and give the student's name and grade level, the doctor's name, and the date and time of the appointment.
The student must sign out prior to departure in the attendance office and at the front desk. Upon returning from the appointment the student must sign in at the front desk and also in the attendance office.
The student is also required to provide the attendance office with the appropriate documentation from the appointment. Until the documentation is provided the absence is unexcused.
Reporting Student Absence
Parents are to call 776-6256 to report a student absence. The call should be made between 7 a.m. and 2 p.m.; if that is impossible, a voice message may be left at any time within 24 hours.
Only married students and those who have been legally emancipated may call in for themselves.
Calls are to provide the parent's name and the relationship to the student, the student's name and grade in school; and the reason for the absence.
According to state law, parents may be required by the school to provide documentation from a medical provider for any student absence.
Absences which are not verified by parents within two days shall be classified as unexcused.
Leaving School Because of Illness
The student must report to the health services office. The nurse will contact a parent. The student must sign out through the attendance office and at the front desk.
Students who leave school without following these procedures are truant.
